Forms: UCC

Washington accepts the national model forms available on the International Association of Commercial Administrators website. All of the following forms are national forms, except the UCC-11 which is a Washington State form.

How to use these forms

To submit a file or search by mail:

1. Select the appropriate form below.
2. Complete the form, following the instructions on the last page of each form.
3. Mail completed forms with the fee to:

UCC forms

• UCC Financing Statement (UCC1)
  Use to file an original financing statement, agricultural, preparer, or processor lien.
• UCC Financing Statement Addendum (UCC1Ad)
  Use to add more information to your UCC-1 financing statement. Cannot be filed separately.
• UCC Financing Statement Additional Party (UCC1AP)
  Use to add additional party names to a UCC-1. Cannot be filed separately.
• UCC Financing Statement Amendment (UCC3)
  Use to change, terminate, continue or assign an existing financing statement.
• UCC Financing Statement Amendment Addendum (UCC3Ad)
  Use to add more information to a UCC-3. Cannot be filed separately.
• UCC Financing Statement Amendment Additional Party (UCC3Ap)
  Use to add additional party names to a UCC-3. Cannot be filed separately.
• UCC Statement of Claim (UCC5)
  Use to dispute an existing financing statement. Alert: don’t use to amend an existing financing statement.
• UCC Information Request (UCC11)
  Use to search the UCC records.
