How to apply for a license

Send us all of the following:

• A completed Promoter/Event License Application
• The fee (check or money order payable to “Washington State Treasurer.”)
• A Professional Boxing, Martial Arts, and Wrestling Surety Bond in the amount required.
• A certificate of medical insurance in the amount required.
