How to change your name
There is no fee to change your name on a license, but there is a $5 fee for a new license that reflects the name change.

Change your name by mail
Gather all of the following:
- Your previous name
- Your current name
- Your new address and phone number (if applicable)
- Type of license
- License number
- A check or money order (payable to the Department of Licensing) for the $5 print fee (if you want a new license)

How to change your mailing or business address
There's no fee to change your mailing address, but you will have to pay the appropriate license application fee to change your business location address. If you would like a copy mailed to you with the name change, there is a $5 print fee.

Contact us and include the following:
- Name
- Current address and phone number
- Type of license
- License number
- For salon/shops, include your business name and Unified Business Identification (UBI) number or tax registration number

Change your business location address by mail
Complete a new license application. If it's time to renew your business license, you can update the location address during the renewal process by paying a renewal or late renewal fee and no additional fee for changing the business location address.
Mail your form and a check or money order for the application fee (payable to the Department of Licensing) to:
