Driver Information Request

The Driver Information Request form is intended for official use only by courts and prosecutors requesting Certified Copy of the Driving Record (CCDR) packets, or certified photographs for use in criminal investigations.
You may:
- complete the form online, save the form to your computer, and email it to DOL as an attachment (if you have the correct software).
- complete the form online, save the form to your computer, and fax from your computer to ours (if you have the correct software).
- complete the form online and print a hard copy that you can fax or mail.
OR - print the blank form, fill it in by hand, and fax or mail it to DOL.
Please don’t use this form to request photomontages or non-certified photographs.. Those requests should be made directly to Drivers Special Investigations (DSI) as follows:
