Use the Recommendation for Driver Reexamination form to report an unsafe driver.
Requirements
Recommendations for driver reexamination must:
- Be made in the interest of public safety only, without consideration to the age of the driver.
- Include specific information about the medical conditions, visual conditions, and driving abilities of the driver.
Based on the information you provide, we’ll investigate and take action as necessary. The information will also be given to the driver or the driver’s attorney if they request it. However, due to privacy rules, we can’t tell you the outcome of the investigation.
Instructions

- Provide as much specific information as possible.
- Include all supporting documents with the form.
