Commercial Driver Fitness Determination
Under standards set by the federal Department of Transportation, you must pass a physical examination given by a medical doctor before you can:
- get a commercial driver license (CDL).
- get a CDL instruction permit.
- renew a CDL.
- transfer a CDL.
The doctor who performs your physical examination must complete this form to certify that you do not have any physical or mental conditions that could affect your ability to safely operate a commercial motor vehicle.

Medical waiver
If you do NOT meet the fitness standards for licensing, you may apply for a medical waiver:
- for intrastate commerce by completing the Commercial Driver License Intrastate Medical Waiver Application.
- for interstate commerce by calling the Federal Motor Carrier Safety Administration at (360) 753-9875.
