National Driver Register (NDR) File Check
What is the National Driver Register?
The National Driver Register (NDR) is a nationwide database of information about individuals whose privilege to drive has been revoked, suspended, canceled, or denied, or who have been convicted of serious traffic-related offenses.
The records provide information about actions reported in the last 3 years, including the:
- Driver’s name, date of birth, and gender.
- Driver license number.
- State where the action was taken.
The records don’t include detailed information to confirm identity or the contents of a driver record. This information is available only from the state where the action was taken.

How to check your own record
- Complete the front side of the National Driver Register File Check Request form and print it on white paper.
- Notarize the form by signing it or having your mark witnessed in the presence of a notary public.
- Mail the completed form to:

How to check the record of a current or prospective employee
- The employer or the driver completes the National Driver Register File Check and Abstract of Driving Record Request for Current or Prospective Employee form and print it on white paper.
- The driver certifies his or her identity, and authorizes the request by signing the form in the presence of a notary public.
- The employer or the driver mails the request and $10 fee to:
