Driver’s Hearing Request and Hearing Fee Waiver Application
This 2-page document includes both the Hearing Request Form and the Hearing Fee Waiver Application form.
Use this form to request a hearing if you have been notified that your driver license is going to be suspended or revoked for refusing a blood or breath alcohol test, or for having a blood alcohol content above the legal limit. Your hearing request must:
- be postmarked within 30 days of the date of arrest.
- include a check for $200 payable to “Washington State Department of Licensing.”
Note: The $200 fee may be waived if you are determined to be indigent.
