For Hire/Limousine Addendum
This form is used to apply for a For Hire or Limousine license.
For Hire license
If applying for a For Hire license, submit the For Hire/Limousine Addendum with the 
appropriate fees and the following forms:
- Master Business Application
- Weighing and Measuring Device Addendum. (To register any measuring device, such as taxi meters.)

Limousine license
If applying for a Limousine license, submit the For Hire/Limousine Addendum with the appropriate fees and the following forms:
- Master Business Application
- Physical Examination
- A copy of the vehicle registration for each vehicle.
- Certificate of Insurance

Please fill out the form completely and print it on white paper. Send your completed application to:
