Personal/Criminal History Statement (Vehicle/Vessel Dealer, Wrecker, or Commercial Telephone Solicitor)
This form is required when applying for a Motor Vehicle Dealer, Vessel Dealer, Wrecker or Commercial Telephone Solicitor license.
Motor vehicle dealer
The Personal/Criminal History Statement must be submitted with the:
Master Business Application- Vehicle Dealer Manufacturer Addendum
- Vehicle Dealer Bond
- AND
- appropriate fees
Depending on your type of business structure, you must submit the following completed Personal/Criminal History statements:
- One for the owner of a Sole Proprietorship.
- One for EACH partner of a General Partnership.
- One for EACH officer, director or shareholder of a Corporation.
- One for EACH partner, member and manager of a Limited Partnership, Limited Liability Corporation, or Limited Liability Partnership.
Commercial telephone solicitor
The Personal/Criminal History Statement must be submitted with the:
- Master Business Application
- Commercial Telephone Solicitor Supplemental Information form
- AND
- appropriate fees
Depending on your type of business structure, you must submit the following completed Personal/Criminal History statements:
- One for EACH manager in charge of business operations.
- AND
- One for the owner of a Sole Proprietorship.
- One for EACH partner of a Partnership.
- One for EACH officer of a Corporation.
- One for EACH member or manager of a Limited Liability Company.
