Master Business Application Supplement for Collection Agency, Branch Office, and Out-of-State Collection Agency Office
This form must be submitted with the 
appropriate fees and the following forms and information:
- Master Business Application
- Location Addendum
(Only if applying for more than one location) - Financial Statement
(Financial statement within the last 3 months, showing liabilities and assets of $7,500, of which $5,000 must be cash and $2,500 can be cash or equivalent.) - Collection Agency or Out-of-State Collection Agency Surety Bond
(This form must be completed by your insurance company, and they must attach their power of attorney document to the form.) - List of persons with more than 20% interest in the business.
