Assessment/Treatment Report for DOT Positive Test
The Assessment/Treatment Report for DOT Positive Test form is used to:
- Report a drug/alcohol assessment.
- Recommend treatment.
- Verify that a driver is satisfactorily participating in, or has successfully completed, drug/alcohol treatment or education.
We must receive a completed assessment/treatment report before we can release a Commercial Driver License (CDL) disqualification resulting from a positive Department of Transportation (DOT) drug/alcohol test or a refusal to take the test.
Instructions
- The report must be completed and sent to us by a DOT-qualified substance abuse professional (SAP).
- All forms must be printed on white paper.
