Professional athletics participants

Cover Letter and Certification of Physical, which must be included with applications for licensing as boxers, boxing referees, martial arts participants, and wrestling participants.
Professional Athletics License Application for licensing as a referee, physician, judge, boxer, martial arts participant, matchmaker, announcer, inspector, wrestling participant, second, timekeeper and manager.

Advance Notice: The promoter is required to submit a completed "advance notice" form no later than seven days prior to an event.
Closed Circuit Telecast License Application: Required before conducting closed circuit telecasts of events.
Contract Between Promoter and Boxing or Martial Arts Participant: This form must be used for contracts between the promoter, manager, and participant.
Professional Boxing, Martial Arts, and Wrestling Surety Bond: Required to get a license.
Promoter/Event License Application to hold a boxing, wrestling, martial arts or closed circuit telecast events.
