Licensing requirements
You must be at least 18 years old to be licensed.
How to apply for a license
You can apply online or by mail.

Create or use your SAW account to apply and pay your fees online
By mail
Send the following:
- A completed Combative Sports Participant License Application/Renewal
- Physical Examination for Referees and Professional Combative Sports Participants, completed and signed by an MD, DO or ND only.
- A check or money order for the application fee (payable to the Department of Licensing).

Terms we use
- Boxing
- A sport of attack and defense which uses the contestants' fists. Contestants compete with the intent to win by decision, knockout, or technical knockout, not to injure or disable an opponent.
- Kickboxing
- A type of boxing in which blows are delivered with the fist and any part of the leg below the hip, including the foot. Contestants compete with the intent to win by decision, knockout or technical knockout, not to injure or disable an opponent.
