How to renew

By mail

• Complete the Reduction Operator Renewal Application.
• Mail renewal application with a check or money order for the application fee (payable to the Department of Licensing) to:
  Funeral and Cemetery Board
  PO Box 3777
  Seattle, WA 98124-3777
• You must include a certificate of completion of operator training if it's been 5 years since you were last certified.

Please allow 5 days, not including mailing time, to process your application.
