Before you get started
- Remember to enclose all necessary documents and fees with your completed application. Your license may be delayed if your application is incomplete.
- Be sure to contact our office, in writing, with any changes to your address.
Instructions
- Fill out the Employment Agency General Manager Examination Application form completely, print it on white paper, and sign it.
- Submit the completed form with all of the following:
- A completed Business License Application (Department of Revenue)
- All appropriate fees
- Send your completed applications and fees to:
